In order to join the program, please fill out the forms listed below, and mail them to mahal@mahal-idf.org no longer than 6 months before the beginning of the program
You should bring with you the originals of the following when coming to Israel, in addition to sending their copies during registration:

A detailed request for volunteering and a detailed personal information questionnaire in Hebrew or English. |

|
A medical declaration filled out by yourself and by your family physician abroad. |
|
|
The physician is required to note in detail any health problems. |
|
|
An original certificate of Judaism from a recognized rabbi of your community abroad. |
|
|
In addition, citizens of the former Soviet Union should approach “NATIV” in order to get confirmation that they are Jewish (This procedure might take as long as 4 months). |

|
An original birth certificate. |
|
|
A passport valid for a minimum of 3 years from the beginning of the program, and at least 2 years from the date of recruitment to the IDF. |
Visa
|
|
When arriving in Israel you should have a valid A2 visa in your passport, signed by the Ministry of Interior. |
|
|
Citizens of an Arab state may experience difficulties getting a visa. |
|
|
An A2 visa is valid for 3 months. During the program we'll take care of extending it by 8 additional months. |
|
|
The letter to the Interior Ministry re extending the visa will be issued by the Ministry of Defense only on condition that you have provided them with the above-mentioned documents. |

|
During service you will have to renew the visa yourself. |
Additional forms
|
|
A request-package for participating in the MDA program, which includes a registration form, a medical statement, a list of vaccines received, passport pictures, a recommendation and a letter of intentions. |

|
|
A letter of intentions for the insurance company. |
|
|
A contract agreement with MAHAL 2000. |
|
|
